As a result,<strong> the CARE Program will be temporarily pausing acceptance of new referrals effective June 1, 2026.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Important points for referring providers:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Referrals submitted after June 1st will receive a standard response indicating that the program is temporarily paused while we work through the current backlog. A waitlist will NOT be maintained during this period.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Referrals received prior to June 1st will continue to be accepted and processed as usual, including referrals that were initially submitted incomplete before June 1 and for which additional information was requested.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Incomplete referrals that remain outstanding without the required information for more than 4 weeks will be automatically cancelled.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We appreciate your understanding and continued support as we work to ensure safe, timely, and high-quality care for patients currently within the program while navigating limited resources and staffing.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Please continue to check the CW OHT website for further updates regarding the anticipated resumption of program. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>What is the CARE Program?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">CARE stands for Coordinated, Accessible, Respectful, Equitable.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The CARE program is designed to provide older persons experiencing frailty with short-term care and support by a health care team in their home. The care team includes a doctor, nurse, and a home care coordinator. This service may also include other health care providers and staff from community agencies based on the patient\u2019s needs.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>What are the goals of the CARE Program?<\/strong><\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Support people to remain in their home as they require additional medical care.<\/li>\n\n\n\n<li>Encouraging people to live independently<\/li>\n\n\n\n<li>Support friend and family caregivers<\/li>\n\n\n\n<li>Answer questions and address their fears and worries<\/li>\n\n\n\n<li>Link people to supports in the community<\/li>\n\n\n\n<li>Make sure information is shared within the CARE program team<\/li>\n\n\n\n<li>Reduce the number of times people may need to go to a hospital emergency department<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>What services are available from the CARE Program?<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">After completing an initial assessment, the CARE team will then work with the patient to create a care plan together. Once the care plan is created, the patient\u2019s care is provided in their home by the CARE program doctor, nurse or any other community agencies or homecare as needed. More details are available in the question-and-answer document here.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Eligible for the CARE Program?<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Eligibility for referral to the CARE program includes all of the following:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Age 65 years or older.<\/li>\n\n\n\n<li>Have difficulty in completing at least one basic daily activity (such as bathing \/ showering, dressing, toileting, walking, or eating).<\/li>\n\n\n\n<li>Have difficulty leaving the home to attend medical appointments.<\/li>\n\n\n\n<li>Home postal code begins with L6P, L6R, L6S, L6T, L6V, L6W, L6X, L6Y, L6Z, L7A, M9R, M9V, or M9W.
