{"id":7316,"date":"2025-10-10T12:01:09","date_gmt":"2025-10-10T16:01:09","guid":{"rendered":"https:\/\/centralwestoht.ca\/?p=7316"},"modified":"2025-10-10T17:14:50","modified_gmt":"2025-10-10T21:14:50","slug":"acknowledging-world-hospice-and-palliative-day","status":"publish","type":"post","link":"https:\/\/centralwestoht.ca\/fr\/acknowledging-world-hospice-and-palliative-day\/","title":{"rendered":"Journ\u00e9e mondiale des soins palliatifs"},"content":{"rendered":"<p class=\"wp-block-paragraph\">Chaque ann\u00e9e, la Journ\u00e9e mondiale des soins palliatifs est comm\u00e9mor\u00e9e le deuxi\u00e8me samedi d\u2019octobre afin de promouvoir des soins compatissants, dignes et accessibles aux personnes atteintes de maladies limitant l\u2019esp\u00e9rance de vie.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">L&#039;\u00c9quipe Sant\u00e9 du Centre-Ouest de l&#039;Ontario (\u00c9SO Centre-Ouest) s&#039;engage \u00e0 garantir \u00e0 tous les r\u00e9sidents de notre r\u00e9gion l&#039;acc\u00e8s \u00e0 des soins palliatifs de qualit\u00e9 et empreints de compassion. Gr\u00e2ce \u00e0 des partenariats avec des maisons de soins palliatifs comme <a href=\"https:\/\/dlhospice.org\/\">L&#039;hospice Dorothy Ley<\/a> et <a href=\"https:\/\/hospicemississauga.ca\/\" data-type=\"link\" data-id=\"https:\/\/hospicemississauga.ca\/\">Hospice Mississauga<\/a>, nous travaillons ensemble pour am\u00e9liorer les soins et le soutien aux personnes et aux familles confront\u00e9es \u00e0 une maladie limitant l\u2019esp\u00e9rance de vie.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Le <strong>Plan pluriannuel de soins palliatifs de CW OHT<\/strong> est une initiative cl\u00e9 et coordonn\u00e9e con\u00e7ue pour am\u00e9liorer l&#039;acc\u00e8s et la qualit\u00e9 des soins palliatifs, y compris des services tels qu&#039;un coach clinique d\u00e9di\u00e9, un tableau de planification des soins palliatifs, un responsable des soins primaires d\u00e9di\u00e9, une ligne d&#039;assistance t\u00e9l\u00e9phonique 24h\/24 et 7j\/7, des parcours de soins int\u00e9gr\u00e9s et une collaboration entre les h\u00f4pitaux, les \u00e9quipes communautaires et les soins primaires. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cela soutient l\u2019accent mis par le CW OHT sur les soins centr\u00e9s sur le patient et la planification de fin de vie.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ce travail est r\u00e9alis\u00e9 en collaboration avec de pr\u00e9cieuses organisations partenaires de CW OHT, notamment <a href=\"https:\/\/www.williamoslerhs.ca\/en\/index.aspx\">Syst\u00e8me de sant\u00e9 William Osler<\/a>, <a href=\"https:\/\/dlhospice.org\/\">L&#039;hospice Dorothy Ley<\/a>, <a href=\"https:\/\/www.ontariohealth.ca\/\">Sant\u00e9 Ontario<\/a>, <a href=\"https:\/\/www.brameast.ca\/\">Organisation de sant\u00e9 familiale BramEast<\/a>, <a href=\"https:\/\/www.wellfort.ca\/\">Services de sant\u00e9 communautaire de Wellfort<\/a>, <a href=\"https:\/\/pchs4u.com\/\">Services de sant\u00e9 communautaire du Pendjabi<\/a>, <a href=\"https:\/\/www.queensquarehealth.ca\/\">\u00c9quipe de sant\u00e9 familiale Queen Square<\/a> et <a href=\"https:\/\/peelregion.ca\/\">R\u00e9gion de Peel<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ensemble, nous esp\u00e9rons r\u00e9duire les visites inutiles \u00e0 l\u2019h\u00f4pital et proposer des parcours de soins int\u00e9gr\u00e9s qui am\u00e9liorent la coordination.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Ressources<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/www.chpca.ca\/knowledge\/resources\/\">Association canadienne de soins palliatifs (acsp.ca)<\/a><\/p>","protected":false},"excerpt":{"rendered":"<p>Each year, World Hospice and Palliative Care Day is commemorated on the second Saturday of October to promote compassionate, dignified, and accessible care for individuals living with life-limiting illnesses.
