Collaborative Effort of Central West OHT, WellFort Community Health Services and Rexdale CHC Improves Health Equity and Access
The teams behind the collaborative Cervical Screening Model have been honoured with the prestigious Quality and Innovation Award from the Cancer Quality Council of Ontario (CQCO).
This recognition celebrates our commitment to advancing health equity and improving cervical cancer screening rates in underserved communities. The initiative has achieved remarkable success in addressing barriers to screening, including structural access, cultural and religious considerations, and socioeconomic challenges.
Key Achievements:
- A 12% increase in screening rates in target communities.
- A 24% improvement in the most under-screened neighborhood in the province.
- A 3% overall population health improvement across the Central West OHT.
“This achievement highlights the incredible impact of collaboration and shared commitment to improving health equity across our communities,” said Rishika Thakur Malhi, Executive Lead of the Central West Ontario Health Team. “By working hand-in-hand with WellFort Community Health Services and Rexdale CHC, the Central West OHT has helped to facilitate the collaborative spirit and innovation that not only breaks down barriers to cervical cancer screening but also builds trust and ensures culturally responsive care. Together, we’re creating the conditions for a healthier future for all.”
Through a shared vision and data-driven strategies, this initiative has transformed access to preventive healthcare, building trust and fostering culturally responsive care within communities.
