Expression of Interest

Strong, sustainable partnerships are critical to the success of the Central West OHT, as outlined in our mission, “Collaborative partners building a healthier community”. The OHT’s current collaborative partnerships advance and support our integration agenda, bringing seamless care closer to home for the communities we serve. The requirements for becoming an OHT partner organization include:

Delivery or support of health or social services in Brampton, Bramalea, Malton, West Woodbridge and/or or North Etobicoke.
Commitment to support OHT initiatives that advance care planning and delivery for the CW OHT’s attributed population, seniors.
Demonstrated alignment with OHT’s strategic priorities, outlined in the 2025-2027 Strategic Plan and 2024-2025 Annual Business Plan.
Signing the active Collaborative Decision Making Arrangement (CDMA) Framework and any related documents (without seeking revisions to those documents).
