Founded in 1982, CANES Community Care is a Not-For-Profit organization that provides a variety of Care Services for seniors living in their own homes, providing emotional and social support, while helping them take part in the life of their community.

Canadian Addiction Treatment Centres was founded with the mission to expand access to evidence-based addiction treatment for people suffering from opioid use disorder. They offer a comprehensive care model designed to meet the broader health care needs of patients. In addition to providing Methadone and Suboxone® assisted therapy, we offer harm reduction services, basic primary care, Hepatitis C and HIV Treatment, and counselling/case management.

Central Brampton Family Health Team (CBFHT) provides patients with comprehensive and continuous care. Our multidisciplinary team is comprised of Family Doctors, Nurse Practitioners, Registered Nurses, Registered Practical Nurses, Mental Health Counsellors, Registered Dietitians, Pharmacists, Respiratory Therapists, and Registered Kinesiologist.

CMHA Peel Dufferin champions good mental health for everyone and supports the full participation of those with mental illness and addictions in the life of the community. In addition to providing a variety of mental health and addictions services, we are committed to community development, mental health awareness and addressing the stigma and discrimination surrounding mental illness.

Home and Community Care Support Services Central West is one of 14 Home and Community Care Support Services organizations in Ontario with a focused mandate to deliver local health care services such as home and community care, long-term care home placement and help finding services and local doctors.

The Dorothy Ley Hospice is a volunteer-based community service organization offering compassionate care to people living with the challenges of a life-limiting illness or loss. Our programs meet their physical, emotional, spiritual and psychosocial needs, helping them to live life to the fullest. Our services are provided at no cost to the individual thanks to the generosity of our donors and community partners.

ESS Support Services is a not-for-profit community support agency that has been supporting older adults, seniors, older adults with disabilities, families and caregivers in the Etobicoke community, and beyond. ESS offers a diverse range of high-quality services and accredited programs designed to support the independence, social well being and health of our clients, along with a helping hand and respite for their families and caregivers.

Heart House Hospice support programs include hospice counselling, spiritual care, HUUG children’s program, day program, health and wellness, education, bereavement and will soon be grown to include a guest home and beds. They serve over 1,000 individuals a year who are living with a life limiting illness and additionally their family members and loved ones.

Holland Christian Homes is committed to providing a supportive, secure and caring environment that encourages independence, preserves each person’s dignity, and promotes a high quality of life for seniors. We offer a continuum of care from independent living apartments to assisted living to long-term care.

At Humber, we offer our students a polytechnic education. We teach theory, add in practical, hands-on training and work with employers to give students opportunities to learn in real-world settings. Set in state-of-the-art and creative learning spaces, this unique approach unleashes innovative and entrepreneurial thinking and prepares students for the careers of today and tomorrow.

North Peel Family Health Team is committed to providing the Brampton community with comprehensive and accessible healthcare effectively and efficiently. And we believe that begins with support—from the right people, with the right skills and expertise, delivering care at the right time.

PAARC offers a variety of programs and services for people experiencing challenges related to their own or someone else’s substance use, gambling, gaming or other select behavioural dependencies.  PAARC also has programs for individuals who are living with mental health challenges

Peel Senior Link was established in 1991 as a local initiative of the Ministry of Health, Halton-Peel Long-Term Care Area Office. Since that time, the agency has grown tremendously, now offering services to buildings and their surrounding neighbourhoods that house over 1,200 seniors in the Region of Peel (Brampton & Mississauga).

Punjabi Community Health Services (PCHS) has a mandate to provide health, settlement and social services to diverse communities in Greater Toronto Area. PCHS also undertakes community development projects to build neighbourhoods and special projects like building “community hubs”.

Queen Square Family Health Team provides accessible, equitable, patient-centred primary care to its local community. A talented, interdisciplinary team works collaboratively with Queen Square Doctors, and together, we serve 35,000 patients annually. We offer a wide range of programs and services including diabetes care, seniors’ wellness, palliative care support, COVID-19 response, trans and gender health, mental health, maternal and children’s health.

The Region of Peel serves more than 1.4 million residents and approximately 163,000 businesses in Brampton, Caledon and Mississauga. We deliver a wide range of resident focused services across the Region.

Rexdale Community Health Centre delivers integrated, coordinated clinical services through a team of physicians, nurse practitioners, nurses, dietitians, social workers, a physiotherapist, and a chiropodist.

Richview Community Care Services Corporation (RCCSC) is a non-profit, charitable, community agency. We offer a range of home support services to eligible tenants living in the Richview Residence complex and surrounding community.

RootsCS partnered with agencies of the federal and provincial governments, the Region of Peel, the City of Brampton, other community service agencies and businesses to deliver programs and services that helped individuals and families from BAC communities improve their livelihood, and contributed to building stronger, safer communities in the Region.

Services and Housing In the Province is a leading non-profit organization committed to alleviating the pressure caused by popular stigmas surrounding mental health. By providing mental health clients with access to supportive housing and professional health services, we create a safe place where individuals gain the ability to use tools, educational resources, and community connections to improve their overall quality of life.

We are one of Canada’s largest owners and operators of seniors’ living, with high-quality seniors’ residences in Ontario, Saskatchewan and British Columbia comprising independent living, assisted living, memory care, long-term care and specialized programs and services for seniors.

WellFort is a community health centre providing health services to individuals and families across the Region of Peel depending on their unique needs. Programs are also known by many other names: Bramalea Community Health Centre, Four Corners Health Centre, Bloom Clinic, Diabetes Education Program, and Health n’ Smiles. All WellFort programs are committed to providing inclusive and sustainable care that can be used by all members of our communities.

West Park Healthcare Centre helps patients get their lives back by providing specialized rehabilitative and complex care after a life‐altering illness or injury such as lung disease, amputation, stroke, and traumatic musculoskeletal injuries.

William Osler Health System (Osler) is a hospital system comprised of Brampton Civic Hospital, Etobicoke General Hospital and Peel Memorial Centre for Integrated Health and Wellness. Osler operates a unit at the Reactivation Care Centre at the Humber River Hospital – Church site and a Withdrawal Management Centre in Brampton.
