On behalf of the Collaboration Council, we are pleased to present the CW OHT inaugural Strategic Plan, a visionary roadmap that has been developed through extensive collaboration with a number of valued partners within our community.
The plan officially launched on April 1st, 2024 and is designed to guide our organization through a transformative journey over the next three years.
We are committed to maintaining transparent communication by consistently sharing updates on our progress in achieving the strategic objectives, fostering a culture of accountability, and ensuring our collective success.

Annual Business Plan – FY 26/27
The Annual Business Plan outlines our priorities for FY 26/27, focused on our big three priorities for this year, in alignment with provincial direction and needs of our local community. These large-scale priorities build on and complement ongoing initiatives, including operating the SCOPE and CARE programs, improving patient engagement, and evolving how we apply an equity, diversity and inclusion lens across all the work we do.
