Acknowledging World Hospice and Palliative Day

Each year, World Hospice and Palliative Care Day is commemorated on the second Saturday of October to promote compassionate, dignified, and accessible care for individuals living with life-limiting illnesses.

The Central West Ontario Health Team (CW OHT) is committed to ensuring that everyone in our region has access to high-quality, compassionate palliative care. Through partnerships with hospices such as The Dorothy Ley Hospice and Hospice Mississauga, we are working together to improve care and support for individuals and families facing life-limiting illness.

The CW OHT’s Palliative Care Multi-Year Plan is a key, coordinated initiative designed to enhance access to and quality of palliative care including services such as a dedicated clinical coach, a palliative care planning table, a dedicated primary care lead, a 24/7 helpline, integrated care pathways, and collaboration between hospitals, community teams, and primary care.

This supports the CW OHT’s focus on patient-centered care and end-of-life planning.

This work is being done alongside valued CW OHT partner organizations including William Osler Health System, The Dorothy Ley Hospice, Ontario Health, BramEast Family Health Organization, Wellfort Community Health Services, Punjabi Community Health Services, Queen Square Family Health Team and Peel Region.

Together, we hope to reduce unnecessary hospital visits, provide integrated care pathways that improve coordination.
