The Central West Ontario Health Team (CW OHT) is pleased to welcome St. Leonard’s Place Peel (SLPP) as its newest partner, further strengthening a collective commitment to person-centered, coordinated, and equitable care in the Central West region.
SLPP provides transitional and supportive housing, health promotion, and reintegration programs for men experiencing situations of homelessness, justice involvement, mental health issues and more.
With the organization’s deep community roots and shared values of accountability, responsiveness, compassion, innovation, and equity, this partnership aligns closely with CW OHT’s mission to create a more connected and accessible health system for all.

The CW OHT looks forward to collaborating with SLPP on system-level strategies that improve care experiences, support integrated service delivery, and address gaps in access for individuals facing housing instability, mental health challenges, and other social determinants of health.
