The Central West Ontario Health Team (CW OHT) is proud to share its 2024–25 Annual Report, highlighting the progress we’ve made together to strengthen integrated, person-centred care across Brampton, Bramalea, Malton, West Woodbridge, and North Etobicoke. Over the past year, the CW OHT advanced its strategic plan through initiatives that improved access, equity, and system navigation for our diverse and rapidly growing population.
While celebrating numerous achievements in advancing patient care (highlighted on page 3), this year also marked important steps in building capacity for the future.
Looking ahead, we remain committed to ensuring that every resident of our region is connected to a primary care provider, advancing health equity, and deepening partnerships with patients, caregivers, and communities.
