The Central West Ontario Health Team (CW OHT) is proud to share its 2025-26 Annual Report, highlighting the progress we’ve made together to strengthen integrated, person-centred care across Brampton, West Woodbridge, North Etobicoke, and Malton. Over the past year, the CW OHT advanced its key priorities through initiatives that improved access, equity, and system navigation for our diverse and rapidly growing population.
While celebrating numerous achievements in advancing patient care (highlighted on page 3), this year also marked important steps in primary care attachment.
Looking ahead, we remain committed to ensuring that every resident of our region is connected to a primary care provider, advancing health equity, and deepening partnerships with patients, caregivers, and communities.
