The Central West Ontario Health Team (CW OHT) is pleased to announce that Dr. Ria Oommen been selected as the new Primary Care Lead for Older Adults, playing a critical role in fostering collaboration amongst community providers for care for older adults, particularly those experiencing frailty.
About Dr. Ria Oommen
Dr. Ria Oommen is a family physician practicing in Peel and Halton. She completed medical school and family medicine residency training at McMaster University.
Her clinical work primarily focuses on providing care for older populations, as a hospitalist at Brampton Civic Hospital (BCH) and attending physician for a Region of Peel nursing home. She has also worked in various community settings, including Community Health Centres, home visits and community palliative care.
She holds faculty appointments with McMaster and Toronto Metropolitan University, serves as BCH’s Corporate Medical Education Lead for hospitalist medicine, and is actively involved with teaching medical students and residents.
Her interest in care of the elderly drives her dedication to provide compassionate, comprehensive care across a variety of clinical settings.
About the Role
The Older Adults Lead acts as Co-Chair for the CW OHT’s Older Adults Planning Table and leads the development, implementation and evaluation of a multi-year plan for Older Adults.
The role includes championing integrated initiatives in the plan and other older adult programs already implemented, such as the CARE Program, which provides wrap-around interdisciplinary care to home bound older adults experiencing frailty.
The lead will leverage their clinical insights to lead OHT partners alongside the OHT Project Manager and key OHT partners, as needed. The lead will also be a key liaison with the CW Primary Care Network and its council.
