What is an Ontario Health Team?

The Ontario government is building a connected health care system centered around patients, families and caregivers, called Ontario Health Teams (OHT).

OHTs organize and deliver services in local communities. They provide people with better connected, integrated and more convenient care.

The goal is to ensure that everyone in Ontario can benefit from better coordinated, more integrated care. Over time, they will work together to make it easier for patients to move from one provider to another, with improvements such as working towards one patient record and one care plan shared between providers.

OHTs are made up of providers and organizations including:

  • primary care
  • hospitals
  • home and community care
  • mental health and addictions services
  • community support services
  • long-term care

OHTs play an important role in mobilizing their partners to respond to local, regional and provincial priorities. They first demonstrated their ability to bring partners together around shared priorities during the COVID-19 pandemic.

This role continues to be important to deal with many system challenges across the province. Examples include managing wait times for services and the expected surge in respiratory illness in the fall.

The teams bring together health care providers from across health and social sectors to work as one collaborative team to better coordinate care and share resources.

The Central West OHT supports a continuum of care with providers in Brampton, Bramalea, North Etobicoke, Malton and Woodbridge.
