Year in Review – 2025

As we approach the holiday season, we are taking a moment to reflect on the important work accomplished in 2025 that our valued partners have helped us achieve to support our community through improved access and stronger attachment to care, enhanced integrated care pathways, and continuously embedding the patient, community and provider voice in all we do.

Over the past year, the Central West Ontario Health Team (CW OHT) has made meaningful progress, including five successful Interprofessional Primary Care Team (IPCT) applications that will help connect more people to primary care and advance integrated care pathways. We are looking forward to expanding our communications around this in the new year.

We had the opportunity to come together through key moments of learning and connection, including our Navigation Supports Conference, the Central West Primary Care Network Annual General Meeting, and our Caregiver Appreciation Event. We also expanded our Patient Family Advisory Council and strengthened palliative care services to name some key accomplishments.

We are also thrilled to have expanded our partnerships by welcoming St. Leonard’s Place Peel, Canadian Red Cross, Toronto Metropolitan University, and Friends & Advocates Peel.

We expanded our team with key roles including the Palliative Care Clinical CoachPalliative Care Primary Care Lead, Patient Attachment Primary Care Lead, Older Adults Primary Care Lead, Older Adults Planning Table Co-Chair and Primary Care Council Specialist.

We launched new resources to help patients understand their care options in our community including:

  • The CW OHT Clinic Map: An interactive map for residents to view their care options throughout our region including walk-ins, pharmacies, Family Health Teams and more.
  • The CW OHT Family Health Team Postcard: A refreshed way to connect patients, providers, and community members to health and wellness offerings across our region.
  • The CW OHT Calendar: A comprehensive calendar of events hosted by providers across the Central West region, easily accessible in one place.

As we close out the year, we hope this season brings you well-deserved time to rest and enjoy moments with loved ones. We look forward to continuing to improve the patient, community and provider voice in all we do in 2026.
